The One Who Meditates on the Lord Crosses Over

ਸਚੁ ਕਰਤਾ ਸਚੁ ਕਰਣਹਾਰੁ ਸਚੁ ਸਾਹਿਬੁ ਸਚੁ ਟੇਕ ॥
sach kartaa sach karanhaar sach saahib sach tayk.
True is the Creator, True is the Doer. True is our Lord and Master, and True is His Support.
ਸਚੋ ਸਚੁ ਵਖਾਣੀਐ ਸਚੋ ਬੁਧਿ ਬਿਬੇਕ ॥
sacho sach vakhaanee-ai sacho buDh bibayk.
So speak the Truest of the True. Through the True One, an intuitive and discerning mind is obtained.

Nanak lives by chanting and meditating on the One, who is pervading within and contained amongst all. ||4||28||98||

In plain words

The one who meditates on the Lord crosses over. The Messenger of Death cannot touch them.

O Nanak — the Lord has given me the Support of the Naam. This is the gift of His Grace.

The crossing over: the Messenger's power is gone. The gift that makes it so: the Naam.

The Naam is the gift of grace that crosses the Messenger's power — what the Saints modeled, the Naam makes available.
The deeper layer

The closing pauri completes the arc from the opening: the Messenger of Death is conquered at the start (through meeting the Saints), and now at the close: jam jam nahi aavai — the Messenger of Death comes not, again and again.

"The Lord has given me the Support of the Naam" — Har naam tika — the Naam as the final support, the one thing that holds. The gift of grace: kirpaa nidhaan — the Treasure of Grace gives what nothing else can.

The crossing is complete. The shabad that began with the Messenger of Death conquered through the Saints ends with the Naam given by grace, which produces the same crossing the Saints had modeled.

A Question to Sit With

What gift have you received that you know you couldn't have given yourself — something that came from outside your own capacity? What was it? What has it meant?
