He is held within every single heart.
Everything that exists is strung like beads on one single thread.
Everything is held together by the One, like beads on a single string.
Sagal samagree jaa kai soot paro-ee — everything is strung on the One's string. Picture a bead necklace. The beads are all different colors and shapes. One thread runs through every bead and holds them together. Take away the thread and they scatter.
And the second line says where the One is found: ghat ghat antar ravi-aa so-ee — the One lives in each and every heart. So the One is the thread through all things, and also inside every one of us.
