Translation by Dr. Sant Singh Khalsa

In plain words

The Supreme One, all-powerful and endless, the First Being, lifts up those who have fallen.

Nanak says: whoever the One lifts up remembers the Creator.

*The One lifts the fallen, and the lifted remember the One.*
The deeper layer

The One lifts up those who have fallen, and those who are lifted remember their Creator.

Patit udhaaran: the One who lifts up the fallen, an old name for the One. Paarbarahm: the Supreme One; samrath purakh apaar: all-powerful, the First Being, without end. Jisahi udhaaray: whoever the One lifts; so simray sirjanhaar: that one remembers the Creator. The lifting comes first, and remembering follows.
