Ego, The Ache Of Feeling Cut Off

The self-willed manmukhs are forever separated from the Lord; no one is with them.
ha-umai vadaa rog hai sir maaray jamkaal.
They are stricken with the great disease of egotism; they are hit on the head by the Messenger of Death.
gurmat satsangat na vichhurheh an-din naam samHaal. ||2||
Those who follow the Guru's Teachings are never separated from the Sat Sangat, the True Congregation. They dwell on the Naam, night and day. ||2||

In plain words

Those who follow only their own minds are always cut off from the One; they wander with no one beside them.

They are sick with the great illness of ego, and death comes and strikes them down.

Those who follow the Guru's teaching are never cut off from the Sangat, the true gathering; day and night they hold the Naam close.

Ego is the ache of feeling cut off — and that ache can be healed.
The deeper layer

Ego is a great sickness that leaves you feeling cut off, and the Guru's teaching and the Sangat keep you close.

Manmukh sadaa vichhurhay: a mind turned only on itself wanders, forever separated and alone. Haumai vadaa rog: ego, the feeling of being a separate self, is the great sickness, and jamkaal, death, strikes such a life down. Ego here is an illness that can be healed. The cure is gurmat satsangat: the Guru's teaching and the Sangat — people gathered together to remember the One — with the Naam held close day and night.
