In plain words

The one who follows only their own mind is always sick with a split, two-sided heart. The whole world is sick with it.

Those who turn toward the Guru understand, and they are healed of the sickness by thinking deeply on the Guru's Word.

The ache of separateness is everywhere, and understanding is the cure.
The deeper layer

The whole world carries the ache of a heart split in two, and thinking on the Guru's Word heals it.

The ache is called dubidhaa, being split in two. It means feeling torn between what we want and the One.

A manmukh is a person who follows only their own wants. The verse says they carry this ache all the time. In fact, rogee sagal sansaaraa: the whole world is sick with it.

Think of a cold going around a school. Almost everyone catches it. Being torn in two is like that. It's everywhere.

But there is a cure. A gurmukh, one who turns toward the Guru, understands what's wrong. Gur sabdee veechaaraa: by thinking deeply about the Guru's Word, the ache heals.

A Question to Sit With

What is something hard that almost everyone feels at some point?
